What is an Action Plan?

An Action Plan is a support document created by the Mentor Teacher and Practicum Facilitator for the purposes of support and intervention on behalf of the TC. It is written once it has been determined that the TC is experiencing difficulty demonstrating the necessary skills, knowledge or strategies for success in the program.

The Action Plan contains:

• The concerns that gave rise to the plan

• What has been done to support to date

• Progress to date and acknowledgement of TC efforts to improve

• Recommendations and timelines

• The nature of the support and how it will be given

• Explicit statement of how progress will be demonstrated

• How and when progress will be reviewed

• Consequences of the failure to improve or make sufficient progress

The Action Plan is written in clear, unambiguous positive language. It is respectful of the TC's struggles, efforts and point of view.

What if a TC is placed in my classroom that has already been identified as being at risk of unsuccessful completion and already has a Learning Plan in place?

There are occasions when a TC who has experienced previous difficulty is placed in your classroom. A consultation with you, MT, Practicum Facilitator and principal would precede this placement. An Action Plan would be in place to support the growth of the TC in the specific areas of need. This information would be shared with you for the purposes of supporting the TC.  While the focus of the Action Plan will differ from TC to TC, the process is the same as outlined above.
